Pre-travel vaccines for Beijing, Shanghai, the Great Wall, Tibet and rural China

China spans more travel-health diversity than almost any country — modern city travel in Beijing, Shanghai or Hong Kong, the Great Wall and historic sites, rural southern provinces, Tibet at altitude, and the Western deserts. The travel-health considerations vary substantially by where you're going.

At Gunwharf Travel Clinic we run China pre-travel appointments year-round. A typical appointment is 30 minutes covering vaccines, region-specific considerations, and any altitude or specialist advice for Tibet.

Abdullah Seyed (MPharm, GPhC reg. 2211356) leads day-to-day clinical practice. Merali Pharmacy, operating Gunwharf Travel Clinic, is GPhC-registered (premises 1099145).

For China most travellers consider: Hepatitis A and Typhoid, an up-to-date Tetanus/Diphtheria/Polio booster, Hepatitis B for longer stays or healthcare exposure, Japanese Encephalitis for rural travel in transmission season, Rabies for extended rural stays, and altitude planning if you're going to Tibet.

China travel-health varies massively by region

A Beijing-Shanghai-Hong Kong city break is a low-vaccine trip. A trek in rural Yunnan is mid-range. A Tibet expedition involves altitude as a primary consideration. We tailor advice to your specific itinerary.

Hepatitis A and Typhoid — universal recommendations

Food and water exposure is real across China including in major cities. Street food in particular is a significant exposure (and part of any decent China trip). Hepatitis A and Typhoid are essentially universal recommendations.

Routine boosters and MMR

Up-to-date Tetanus/Diphtheria/Polio booster. Worth checking MMR status — measles outbreaks occur sporadically in China and unvaccinated UK travellers are at risk. Hepatitis B for longer stays.

Japanese Encephalitis — selective

JE is endemic in southern China rural rice-growing areas, with seasonal transmission peaks during summer wet season. Vaccination worth considering for travellers spending extended time in rural southern provinces (Yunnan, Guangxi, Sichuan basin areas) during transmission season. Two-dose course over 28 days.

Rabies in rural China

Stray dog populations in rural China have rabies risk. Rabies pre-exposure worth considering for travellers in rural areas, especially extended stays or volunteers with animal contact.

Tibet — altitude is the dominant story

Lhasa sits at 3,656m. Many travellers visiting Tibet fly directly from a sea-level Chinese city, hitting altitude without acclimatisation. The result is significant rates of Acute Mountain Sickness for arriving travellers. We discuss: acclimatisation strategy (better is to travel into Tibet by train via Xining which gives gradual ascent over 24 hours), acetazolamide (Diamox) as prophylaxis, recognising warning signs, and descent triggers. We can prescribe Diamox at your appointment.

Air quality — the under-discussed health risk

Northern Chinese cities including Beijing have periodic severe air pollution episodes (PM2.5 levels regularly exceed WHO healthy thresholds many times over). For travellers with asthma, COPD or cardiac conditions, this matters and should be part of pre-travel planning. We discuss N95-rated masks and pollution-tracking apps.

Tap water and food

Tap water isn't safe across China — use bottled or boiled. Hot cooked food is generally safe; raw vegetables and salads are higher risk. Street food at busy stalls with high turnover is reasonably safe; quiet stalls are not.

Common questions

Common China travel vaccine questions

Do I need vaccines for China?

Hepatitis A and Typhoid recommended for almost all travellers. Tetanus/Diphtheria/Polio booster if it's been more than 10 years. MMR check. Hepatitis B for longer stays. Japanese Encephalitis and Rabies depend on rural exposure.

Is malaria a risk?

Very limited. The southern provinces of Yunnan and Hainan have residual focal risk; most tourist areas are malaria-free. We assess based on your itinerary.

What about Tibet altitude?

Lhasa is 3,656m — high enough to cause significant Acute Mountain Sickness in unacclimatised travellers. We discuss acetazolamide (Diamox), acclimatisation strategy (train via Xining if possible), and warning signs.

What about air pollution in Beijing?

Periodic severe pollution episodes. Worth checking real-time air quality (PM2.5 levels) when planning outdoor activity. For travellers with asthma, COPD or cardiac history, this needs specific planning. N95 masks can help.

Is the tap water safe?

No — use bottled or boiled throughout China. Hot cooked food is generally safer than salads or uncooked vegetables.

What about street food?

Busy stalls with high turnover are reasonably safe and part of the experience. Quiet stalls or unfamiliar establishments are higher risk. Hep A + Typhoid vaccines give backup if you do get caught out.

How early should I book?

2–4 weeks for most China travellers. Longer if you need the JE (28-day) or Rabies (21–28 day) courses.

Where do I get my China vaccines in Portsmouth?
